I, hereby agree to allow my child to participate in the youth soccer program offered by Morsink Soccer Academy. In consideration of being allowed to participate in the program, I hereby acknowledge and agree to the following:

 

1. I understand that participation in youth soccer involves certain risks, including but not limited to, those caused by terrain, facilities, weather, equipment, and other participants. I acknowledge that these risks may cause injury, illness, or death.

2. I assume full responsibility for any and all risks of injury, illness, or death that may result from my child's participation in the youth soccer program.

3. I release and discharge Morsink Soccer Academy, its officers, directors, employees, agents, and volunteers, from any and all liability, claims, damages, or causes of action, which may arise from my child's participation in the youth soccer program, including but not limited to, any claims for personal injury, property damage, or wrongful death.

4. I agree to indemnify and hold harmless Morsink Soccer Academy, its officers, directors, employees, agents, and volunteers, from any and all claims, damages, or causes of action, which may arise from my child's participation in the youth soccer program.

5. I understand that this waiver and release of liability is binding on me, my child, my heirs, executors, administrators, and assigns, and that it shall be governed by the laws of the state in which the youth soccer program is located.

 

I have carefully read and fully understand the contents of this waiver and release of liability, and I voluntarily agree to its terms.
